Can you use the nonconforming report for R&D failure or it is simply for production failure only?

It varies from company to company.
You would have to contact the Quality management, or one of the quality engineers to obtain the proper procedures for your company. They can tell you whether it is appropriate to use that report, or if not, which report to use.
