Title of a song from a ways back?

I can't think of the name of the song, but it's about an old man on his deathbed and he's reminiscing on old memories. He talks about being under a tree an I think there's a dirt road, but then his son or grandson wakes him up and he says he was almost home or something. Please help me out. Thanks!

    I think your talking about Almost Home by Craig Morgan. Here are the lyrics:
